Case Management Services
Our goal is to help you to stay healthy. We know that situations change - new problems arise and sometimes old problems return. That is why LCL offers a free service to our Helpline callers - a periodic, brief phone call to verify your present needs are being met and that you have been offered all appropriate services.
By staying in regular contact, we can help you identify and address problems early - before they become serious and cause harm. LCL staff can assist you in coordinating the services you require, answer questions you may have and send you information relevant to your situtation and needs.
Please note, however, these services are optional. If you prefer that we do not stay in touch with you, we will respect your privacy.
